Graft Failure

When a replacement graft fails, the repair is coded either as a revision of the device in the body part or as removal of the device from the body part.  No excision or resection of the native body part is coded, except in a mastectomy.

When a transfer flap graft fails, the repair is coded as excision of the body part location of the transferred flap graft.
